Required Admission Documents

There are a variety of documents we are required to collect for your children.  The forms required for each child will be specific to them and their grade level.  You will see a list of required documents in your TADS account. You can submit them directly to your TADS account, fax them to the school office at 651-481-1355, email them to the school office or drop them off at the school office.  

Please review the information below for the requirements and links for any blank forms you may need.  If you need a blank copy of a form, please select from the list below.

Birth Certificate – We need a copy of the birth certificate for every new student

Dispensation of Medication – To be filled out and turned in with the medication to the front desk. Please stop in the main office of the school for copies of the form. 

Early Childhood Screening – We need a copy of the preschool screening performed for your child. For more information, visit the preschool tab.

Health Care Summary – This needs to be filled out by your doctor.  Preschool, Kindergarten and new students (with the exception of grade 7), are required to submit this.

Preschool/Kindergarten Immunization Form

1st-8th Grade Immunization Form
This form can be filled out, or we also accept a print out from your health chart or doctor’s office showing the immunizations with dates administered.

7th Grade Physical Exam – This form needs to be completed by your doctor.
